El uso de antibiótico en el final de la vida representa un desafío clínico y ético, debido a la tensión entre el alivio sintomático y el riesgo de desproporción terapéutica. En este contexto, el presente estudio tuvo como objetivo analizar críticamente la evidencia disponible sobre el uso de antibióticos en el final de la vida. Para ello, se realizó una revisión narrativa de la literatura entre enero 2018 a diciembre 2025, con actualización enero 2026. Utilizando bases como LILACS, Redalyc, PsicoINFO, PUBMED/MEDLINE, Scopus y SciELO. Utilizando descriptores como cuidados paliativos, cuidado terminal, antibacterianos, inutilidad médica, combinados con booleanos. Se incluyeron artículos a texto completo, en inglés, portugués y español. Se excluyeron publicaciones no relacionada con la temática y duplicados. Se identificaron 42 estudios, solo 32 cumplieron con los criterios de inclusión. En el final de la vida el uso de antibióticos es frecuente y empírico, incluso en ausencia de confirmación microbiológica. Los beneficios clínicos son variables, no se ha develado mejoría consistente sobre la sintomatología o supervivencia. Además, se identificaron riesgos asociados, efectos adversos, incremento de la medicalización y resistencia antibiótica. En este contexto, su indicación debe basarse en una evaluación ética y proporcional, priorizando el confort, la dignidad y los valores del paciente en el final de la vida.
